Instructions for Parents
1. The District will provide a Supplemental Educational Services Enrollment Form and list of all state-approved providers to parents whose children are in eligible schools in September. The enrollment form can also be downloaded from this site.
2. Parents will review the services for each provider, and may contact SES providers for more information. The school may not recommend a provider to parents, but may answer questions about providers when asked.
3. If parents wish to enroll their child(ren) a completed SES enrollment form for each child must be submitted to the school Principal for their signature before the deadline.
4. Parents, the principal, a district official, and the provider will sign and date the Supplemental Services Enrollment form. All forms will be filed in the office of the district before tutoring can begin.
5. After services begin, the parent may change providers if services are unsatisfactory. Before changing, the parent must contact the SES Liaison at 860-823-6284 for instructions on how to proceed.
6. The No Child Left Behind law requires that priority be given to children with the greatest need. Therefore, if large numbers of students request supplemental educational services, not all eligible students may be served, and we may need to give priority to the lowest achieving students. Students not placed on the active list will be placed on the waiting list and moved to the active list when space becomes available.
