Location: The School Based Health Center (SBHC) is located in Kelly Middle School building.
Hours: 8:00 – 3:00 PM Monday – Friday, Open year round
The Kelly Middle SBHC is one of more than 70 similar facilities around Connecticut. It is a freestanding clinic located right on school grounds. SBHCs are licensed by the Connecticut Department of Public Health and provide confidential physical and mental health services to students on site including:
· Physical Exams for sports, school, camp
· Immunizations
· Care of acute illness
· Health promotion and education
· Individual and family counseling
· Parent education
All children enrolled at the site school may use the SBHC regardless of income or health care coverage. SBHCs work in collaboration with local advisory boards composed of local medical health professionals, school officials, community leaders, parent, and students.
In addition to enrollment in SBHC, it is important for the children to be registered with a primary care provider for continuity of care and for evening and weekend coverage.
Parents must sign a permission form before the child can be seen. Permission forms are available at the school nurse's office, the main office or the SBHC. Parents may always call the SBHC and request an appointment for their child. The SBHC staff can be reached at 860-885-1587.
