State of Connecticut Immunization Requirements - NEW!

 

Changes to Immunization requirements for school entry are now official!  Please read below to learn how this will impact your student:

 

Grade 6 students:  All grade 6 students should submit to the TMMS Health Office a completed Physical Exam (blue HAR form) by July of 2011.  If you have already done this, thank you!  In addition, you should check to see that your six grade student has received, or will receive, three specific immunizations which will be required for entry into grade 7 in Sept 2011.  These immunizations are

 

·    2^nd dose Varicella vaccine (Chicken Pox)

·    Adolescent Tdap vaccine (Tetanus, Diptheria, Pertussis)

·    One dose of Meningococcal vaccine (Meningitis)

 

Grade 7 and 8 students:  In most cases, immunization requirement changes will not affect students entering grades 8-12.  Check with your pediatrician for vaccination recommendations.
